Recommendation Request Form

So you want me to write a letter of recommendation for you. Please be aware that I get a large number of requests for recommendation letters. Good letters take time. I need at least two weeks notice and if it is for a professional school, four weeks is requested.

In order to write a letter, I will need the information below. Besides the basics, the rest of the information requested is completely voluntary. However, I can generally write a better and more detailed letter with more information.

This is a general form for all requests.  Some of the fields may not apply to you.  If this is the case, please leave them blank.  If you are applying to multiple schools (this is mostly for graduate school applications), please list them all in the box asking where the letters should be sent.

Please list your extracurricular, volunteer and other relevant activities. A list of grades would also be helpful (but is not necessary), especially if your grades in my classes were lower than you would have hoped or if your grades have been improving with more time at Albion College. These pieces of information will give me a more complete picture of your performance at Albion College. Alternatively, you may email me a .doc or .pdf file that contains this information.  Please indicate in the box below if you are doing this.

Where does the letter need to be sent?  Please include the full address if the letter needs to be mailed. If it is an online form, enter ONLINE and enter the website, if you have it.
